A professionally drafted liability waiver for personal training businesses — ready to use as-is or convert to a digital waiver with QR-code signing. No paper, no clipboards, no lost forms.
Personal training is one of the most individualized fitness services — which means trainers carry personal liability that can't always be transferred to a facility. Whether you work independently, rent space in a gym, or train clients in their homes, a signed waiver and health disclosure form is your primary protection against claims arising from the inherent risks of intensive exercise. The waiver documents that clients understood the demands of training before they began.
ASSUMPTION OF RISK AND LIABILITY WAIVER — PERSONAL TRAINING
I, the undersigned participant (or parent/legal guardian signing on behalf of a minor), voluntarily agree to participate in activities offered by [Your Business Name] ("the Company") and fully understand the nature of such activities.
ASSUMPTION OF RISK
I acknowledge that participation in personal training activities involves inherent risks of injury or harm, including but not limited to: muscle strains, joint injuries, overexertion, cardiovascular stress, improper form injuries, exercise-induced medical events. I understand that these risks exist even when activities are conducted with the highest standard of care and supervision.
I have completed a health screening questionnaire and disclosed all medical conditions, injuries, and medications to my trainer. I understand that my trainer will design programs based on my stated fitness level and I agree to communicate any pain or discomfort immediately.
RELEASE OF LIABILITY
In consideration of being permitted to participate in activities, I hereby release, waive, discharge, and covenant not to sue [Your Business Name], its owners, operators, directors, employees, agents, volunteers, and successors (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in such activities, or while on the premises of the Company, WHETHER OR NOT CAUSED BY THE NEGLIGENCE OF THE RELEASED PARTIES.
INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including attorney's fees, that they may incur arising from my participation in activities or resulting from my breach of this agreement.
MEDICAL TREATMENT AUTHORIZATION
In the event of injury or medical emergency, I authorize the Released Parties to obtain emergency medical treatment on my behalf and agree to assume all costs associated with such treatment.
SEVERABILITY
I expressly agree that this waiver is intended to be as broad and inclusive as permitted by the laws of the state in which activities are conducted. If any provision of this waiver is held invalid, the remaining provisions shall remain in full force and effect.

Paper waivers get lost, damaged, and are nearly impossible to search. Digital waivers solve all of that — and provide stronger legal documentation.
Customers scan a QR code and sign on their own device in under 30 seconds. No clipboards, no pens, no waiting.
Find any waiver in seconds by name, date, or email. No filing cabinets, no boxes of paper. Your records are always accessible.
Every digital signature includes a UTC timestamp and IP address — stronger legal documentation than a scanned paper form.
Common questions about personal training liability waivers, digital signing, and legal requirements.
Personal trainers work one-on-one with clients, often pushing them toward their physical limits. Without a waiver, trainers are exposed to claims from common training injuries — muscle strains, joint pain, overexertion events — even when the training was appropriate and professional. A waiver documents the client's informed consent and health disclosures.
Yes — a health screening questionnaire (PAR-Q or similar) should be completed alongside the waiver. The screening documents the client's cardiovascular health, recent injuries, and any conditions that affect training. If a client has a medical event and did not disclose a relevant condition, the documented screening significantly protects the trainer.
Waivers are especially important for home and outdoor training where you have less control over the environment. Your waiver should mention that training takes place in uncontrolled environments and include specific language about outdoor hazards, equipment provided by the client, and the client's responsibility to ensure a safe training space.
Yes. Independent personal trainers who work without a facility can use this template with their business name in place of a gym name. If you train clients at a commercial gym under a separate business arrangement, you may need both the gym's facility waiver and your own trainer waiver.
At minimum annually, or whenever the client's health status changes significantly (new injury, surgery, new medication, pregnancy). The waiver renewal is also an opportunity to update the health screening, ensuring your training program continues to be appropriate for the client's current condition.
