I, the undersigned participant, acknowledge that participation in physical fitness activities at this facility involves inherent risks of injury, including but not limited to muscle strains, sprains, fractures, cardiovascular events, and other physical harm.
I voluntarily assume all risks associated with my participation in gym and fitness activities. I confirm that I am in adequate physical condition to participate and have consulted a physician if I have any pre-existing medical conditions. I agree to follow all posted rules, safety guidelines, and the instructions of staff at all times.
In consideration of being permitted to use the facilities and services provided, I hereby release, discharge, and hold harmless this facility, its owners, employees, agents, and affiliates from any and all claims, demands, damages, costs, or causes of action arising from my participation in fitness activities, including claims of negligence.
I understand that this waiver is binding upon myself, my heirs, assigns, and legal representatives. This waiver shall remain in effect indefinitely unless revoked in writing. I have read this waiver carefully and sign it voluntarily.
